Description:
Utilizes evidence-based practice to support clinical interventions.
Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with physical functioning disorders under the supervision and direction of a Physical Therapist.
Administers various procedures as part of the rehabilitation plan, including but not limited to manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices.
Assesses patient progress and communicates with the supervising Physical Therapist to adjust treatments accordingly in the clinical record.
Reports effectiveness of treatment outcomes to the supervising Physical Therapist.
Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
Effectively communicates with the supervisor and other health team members regarding patient progress, barriers, and treatment plans.
Provides input to the supervisor for discharge summary of services provided.
Documents all regulatory and clinically required information in the electronic documentation system, maintaining patient records according to policy.
Accurately records treatment minutes for services provided in compliance with all regulatory and clinical requirements.
Participates in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.
Engages in facility and company required training, in-services, and conferences.
Works with the supervising Physical Therapist to obtain any necessary durable medical equipment or supplies.
Supervises and supports students under the guidance of the Physical Therapist.
Adheres to practice acts, codes of conduct, and ethics during professional practice as a Physical Therapist Assistant.
Maintains active State Licensure when applicable.
