Description: The role involves providing in-home therapy services to patients. Common patient demographics include adults and primarily elderly individuals receiving Medicare. The focus is on multidisciplinary cases, including rehabilitation and surgical referrals. Duties include managing admissions and following up with existing patients for evaluations and recertifications. Therapists can start their shift from home and will travel within a specified radius for patient visits. Therapists typically see 4-6 patients per day. No additional support staff will be provided for home health. Interdisciplinary meetings are held on designated days to discuss current cases. Charting is done using an electronic system. Business casual dress code is required, with any color scrubs allowed.
