Description:
Actively monitors and amends patient visit schedule to maximize productivity and patient need.
Takes action to account for new patients and changes in existing patient schedule, including, but not limited to, calling patients to reschedule visits, reshuffling patient load to meet new demands, and communicating recommended changes to supervisor.
Performs comprehensive assessments of assigned patients, including psychosocial, educational, safety, and risk factor parameters, as documented in clinical records.
Develops a plan of care to promote optimal functional independence and safety as documented in clinical records.
Creates patient-specific goals and ensures that the plan of care reflects logical interventions and progress toward these goals.
Plans for and provides skilled physical therapy treatment, including therapeutic techniques and patient/caregiver education.
Assesses patient status and progress on each visit to ensure quality of care, as documented in clinical records.
As Care Manager and/or team member, coordinates patient care with other clinical team members.
Supervises and directs clinical activities of PTAs, PT students, and home care aides.
Completes and coordinates required documentation to ensure compliance.
Participates in in-service training programs.
Maintains and enhances personal knowledge and promotes standards in physical therapy.
Engages in the marketing and promotion of rehabilitation services.
Carries out various patient-related health assessments and procedures.
Communicates changes in patient care schedules to scheduler and supervisor in a timely manner.
Fulfills responsibilities to document patient incidents in a timely manner.
Assesses patients' needs for additional services and coordinates plans of care accordingly.
Coordinates safe patient discharges and ensures processing of all appropriate paperwork.
Participates in regular peer review audits and accepts feedback professionally.
