Description: Function independently to provide high-quality patient care in a collaborative and patient-centric manner. Care may occur in various settings and modalities, including direct interaction with patients in the hospital or clinic, and via telephone, video, or secure email when appropriate. Responsibilities include gathering medical history from patients and others, performing physical exams, reviewing prior records, and analyzing ancillary data using sound judgment and critical problem-solving skills. Develop and monitor appropriate treatment plans, communicating effectively with patients and families when relevant. Provide efficient and compassionate care to both patients and families through a multidisciplinary approach that emphasizes communication, continuity, coordination, and evaluation. Educate and counsel patients and families to support care plans, treatment, and discharge processes. Collaborate with other healthcare team members, leadership, and medical staff to ensure safe patient care. Communicate pertinent findings and information to physicians and the care team. Assist collaborating physicians with the management of patients within established policies, regulations, and scope of practice. Ensure compliance with regulatory standards and practice metrics. Document in accordance with regulatory and care guidelines. Promote the mission, vision, and values of the organization while adhering to service behavior standards. Perform other duties as assigned. May be required to float to other departments based on business needs and may also be required to be on-call during scheduled shifts.
