Description: The Cardiac Rehab Coordinator will provide direct supervision for patients receiving cardiac or pulmonary rehabilitation. The Coordinator will immediately respond to any adverse events during the provision of rehabilitation services. This role may involve participation in hospital and nursing committees as directed by nursing leadership. The Coordinator will collaborate closely with Medical Directors to evaluate needs, develop plans of care, and implement care for patients. Coordination of patient care will occur in collaboration with a multidisciplinary team, including exercise physiologists and rehabilitation technicians. The Coordinator will manage all aspects of the multidisciplinary education and training required for rehabilitation services. The Coordinator will ensure that the psycho-social and educational needs of patients are met. Tasks include setting up, monitoring, and documenting rehabilitation services while ensuring compliance with all regulatory requirements. The Coordinator will perform medical assessments, including history and physical assessments, and consult with physicians regarding findings or changes in patient conditions. Ensuring compliance with all requirements concerning the provision of rehabilitation services is critical. The Coordinator will interact closely with referring physicians and may diagnose medical conditions, plan interventions, and evaluate patient responses to therapy in collaboration with the supervising physician. With guidance from medical leadership, the Coordinator will develop individualized exercise and treatment plans. The position requires the ability to obtain patient histories and perform physical examinations. Monitoring the effectiveness of therapeutic interventions is essential. The Coordinator must possess strong problem-solving, organizational, and communication skills. Effective communication and the ability to respond to changes in patient conditions are essential for this role. Implementing ACLS protocols when necessary and utilizing cardiac telemetry monitoring are required skills. Physical ability to work and stand frequently for extended periods is necessary. The role will likely involve standing and walking for the majority of the shift. Proper body mechanics must be utilized at all times. Manual dexterity is crucial for performing designated duties, with both fine and gross motor skills required. This position will entail regular exposure to the risk of bloodborne diseases and body substances.
