Description: Utilize the nursing process in the provision of direct patient care. Responsible and accountable for prescribing, delegating, and coordinating all care provided to assigned patients. Delivery of care is based on the standards of care and practices established by the nursing department and other policies and procedures. Diagnose and treat human responses to actual and potential health problems through services such as case finding, health teaching, health counseling, and provision of supportive care to restore life and well-being. Supervise patient care provided by licensed practical nurses, registered nurses on permit, nursing attendants, technicians, and supplemental staff.

Assess the immediate and continuing care needs of the patient or significant other. Develop comprehensive and current plans of care which include physical needs, patient teaching, discharge planning, and psychosocial/spiritual needs. Provide direct patient care based on assessed needs and the medical regimen. Oversee and ensure the provision of all aspects of care for the patient. Supervise the care provided by other nursing personnel as defined in the scope of practice.

Observe, evaluate, document, and communicate to relevant team members the patient's responses to illness and/or therapeutic modalities. Ensure timely reassessment of patient responses to treatment and when significant changes occur in patient condition and/or diagnosis. Coordinate the overall plan of care to ensure implementation of nursing interventions and consistency with the medical regimen. Establish priorities for care in collaboration with the multidisciplinary team and initiate follow-up as indicated to ensure timely provision of patient care and therapeutic modalities. Assure the plan of care integrates age-specific care needs as defined in the unit scope of care.

Accept responsibility and accountability for nursing interventions and outcomes based on professional clinical judgments. Participate in quality improvement activities related to patient care delivery and the development of nursing standards. Demonstrate the ability to communicate effectively, manage stressful situations, and facilitate interpersonal relationships with other disciplines. Participate in the orientation of nursing staff. Assume charge responsibilities as delegated by the manager or designee.

Ensure appropriate coverage of all patients by assisting and relieving co-workers, and working on other units as assigned. Maintain a safe, therapeutic environment for patients, significant others, and staff. Serve as a clinical resource for student nurses. Participate in meetings, committees, conferences, and educational programs. Participate in research and pilot projects supported by the nursing department.

Demonstrate responsibility for continuing professional growth and education, including attendance at staff development and continuing education programs, pursuing baccalaureate or graduate degrees in nursing, and obtaining appropriate professional certifications. Participate in appropriate professional and community organizations. Perform other duties as requested.

Physical Requirements: Position requires prolonged periods of standing, reaching, and walking throughout the working day. Position will require stooping, bending, lifting, and carrying items weighing up to 10 pounds for routine performance of essential functions. Able to lift up to 100 pounds, if necessary, to assist in lifting and moving patients. An individual in this position may be exposed to adverse indoor and outdoor environmental conditions, blood-borne pathogens, physical hazards, and chemical hazards. Ability to fully utilize electronic record systems and other healthcare and billing systems relevant to this position.
