Triaging emergent situations and remaining independent in the application of advanced nursing knowledge is essential. Monitoring acute and chronic medical conditions in coordination with Primary Care Providers (PCPs) is necessary. Utilizing care plans independently to provide clinic and home-based direct care is expected. Administering treatments, procedures, and medications as prescribed to participants is a key responsibility. Making home visits regularly to provide care and treatment as ordered by the PCP is required. This may include wound care, lab draws, nurse assessments, and other nursing tasks. The role has a visible presence while working and is subject to frequent interruptions. Communicating with staff, participants, caregivers, and other organizations as necessary is important.
