Description:
As a member of an interdisciplinary mental health treatment team, responsible for assigned mental health interventions for a caseload of mentally ill offenders.
Responsible for the completion of screenings that include detailed mental health screens and risk assessments, including suicide watch assessments when determined to be within the scope of practice and allowable per the credentialing board and policy.
Conduct individual and group clinical interventions as prescribed on the individual mental health treatment plan using clinical intervention strategies within the scope of practice and directed by supervision.
Provide psychosocial group interventions as prescribed on the mental health treatment plan.
Provide mental health liaison services to an assigned caseload, including monitoring offender progress with the mental health treatment plan goals.
Complete psychosocial needs and treatment assessments as part of interdisciplinary treatment planning.
Complete transfer and discharge summaries.
Consult with other staff regarding the mental health caseload inmate's institutional adjustment and service needs.
Initiate and maintain contact with families and community agencies.
Provide and seek information relative to patient care and treatment.
Make appropriate community referrals, per the mental health treatment team.
Write reports and/or documentation to include electronic documentation of all inmate contacts, mental health liaison progress note entries, and discharge plans as needed per policy and protocol.
Attend meetings of social work or counselor supervision.
Attend in-service training sessions and workshops for professional development.
Perform other duties as assigned.
