Description: As an integral part of a team, the Virtual Care Coordinator is responsible for supporting virtual patient care activities and promoting patient-focused care. This role involves performing various remote monitoring duties related to patient safety and clinical needs. The coordinator provides support and assistance with telemedicine services and monitors environmental status. Collaboration is essential for implementing planned telehealth and remote patient monitoring programs. The coordinator collaborates with leadership to coordinate training and participates in the development of telehealth best practices. Clear communication and collaboration with the clinical team, patients, and technology partners are critical. The role shares accountability for clinical quality and safety outcomes. Essential duties include performing quality assurance, peer reviews, and evaluating telehealth programs in collaboration with leadership. The coordinator meets regularly with senior leadership to discuss quality metrics and provide feedback on program workflow. Collaboration and case reviews with hospital partners to implement solutions and monitor outcomes are required. The coordinator triages and assigns cases for virtual monitoring and observation. Collaboration on telehealth policy and workflow development and implementation is expected. Maintaining an operational database for telehealth performance and quality metrics is part of the role. Identifying concerning trends and participating in corrective action plans are responsibilities of this position. Working efficiently with telehealth platforms and technology is essential to meet goals. Successful completion of orientation is required. Training new staff on telehealth and remote monitoring workflows is expected. The coordinator serves as a liaison between all stakeholders regarding telehealth platforms. Facilitating camera utilization and coordinating transitions with hospital care teams is required. Navigating multiple technology platforms to ensure quality care is needed. Providing real-time support to troubleshoot telehealth issues for clinical staff is essential. Continuous observation and surveillance of assigned patients to ensure safety are expected. Clinical early warning sign monitoring such as vital signs is part of the role. Effective interpersonal skills for communication and collaboration are crucial. Immediately reporting any concerns in a patient's clinical status to the relevant team is required. Documentation of patient behavior and interventions is critical. The coordinator suggests to the clinical team if a patient can be discharged from remote monitoring based on behavior. Monitoring telehealth consult orders and ensuring sessions begin as scheduled is required. The coordinator greets patients, families, and visitors, introducing themselves and their role. Prompt response to calls from clinical teams is expected. Using approved communication methods with the clinical team is essential. Ensuring patient privacy by adhering to security policies and regulations is required. Escalating patient safety concerns to supervisors and leadership is critical.
